  • Follow the directions and formatting on page 2 of the PSQ closely.
  • If you have a "Yes" response for any of the five questions, please complete the Template form that corresponds to the question.

§  For example, a "Yes" on question #3 of the "Pre-Screen Questionnaire" requires you to complete the form titled "PSQ_T3_Foreign Travel."

  • Sign and date the PSQ and any necessary Templates.  An electronic signature using a DOD CAC is acceptable.  Ensure all of the dates signed on each of the forms match (i.e., all forms need to have the same date).
  • Please return only the Questionnaire and Templates that were filled out.  Ensure you encrypt the email to protect your PII.
  • Mark files with SSNs as CUI.


Clarifying notes:


  • Each instance of foreign travel must be disclosed on a separate PSQ Template 3; more than one trip on a single form is not allowed.

§  Official travel (deployments, TAD) do not need to be reported unless you took personal leave in conjunction with official travel.

  • Foreign assets and a foreign associate are disclosed on the same form (PSQ Template 2).  However, when disclosing more than one foreign associate, the information on the foreign assets is entered onto only one of the PSQ Template 2 forms that is used to disclose a foreign associate.  For example, if you disclose three foreign associates, you will submit three separate forms (one for each associate) and the foreign asset information will appear on only one of the three forms.

§  US based mutual funds with international holdings/benchmarks (i.e. TSP I-fund) do not need to be reported.

  • All unreported associations, travel, etc. must be reported to your SSO/Command Security Manager prior to submitting the PSQ/templates to ensure DISS matches your PSQ and templates.
